Preliminary Questionnaire
Preliminary Questionnaires will be accepted from Friday, July 11th, through Wednesday, August 6th, at 3:00 p.m.
This Preliminary Questionnaire helps us to identify who you are and whether you will meet our organization’s expectations and standards regarding alcohol use, drug use, criminal history, domestic violence, and safe driving. Part of the screening process for a position with the Whitestown Metropolitan Police Department is a thorough background investigation. This questionnaire is the first step in the hiring process for all positions in the Police Department. The information you provide in this questionnaire will be verified through a variety of sources. If it is discovered that you have made false, misleading or inaccurate statements, you WILL be disqualified from the process. If you have been hired before the inaccuracy has been discovered, you WILL be dismissed. Negative information revealed in this questionnaire may or may not be grounds for disqualification, but DISHONESTY IS ALWAYS GROUNDS FOR DISQUALIFICATION!. After review, you will be contacted by a Whitestown Metropolitan Police Department representative to invite you to a Mandatory Orientation meeting, or you will also be notified if you are disqualified.
